Registration Fees and Deadline

• First Registration per member ministry $400
• Each additional registration member ministry $350
• Retired CHHSM CEO / Spouse (full time) $275
• Per day registration (including meals):
  Thursday $150
  Friday $150
  Saturday $175

The deadline for receipt of Annual Meeting registrations is February 13, 2008. Early registration is encouraged.

Cancellation Policy
Cancellations received in the CHHSM office by Friday, January 9 will be refunded at 100% of the registration fee. Cancellations received between January 10 and the close of business at the CHHSM office on Friday, January 25, 2008 will be refunded at 50% of the registration fee. No refunds will be issued after January 25. However, registration is transferable upon notification to the CHHSM Annual Meeting registrar, Lorraine Cogan at (216) 736-2250.

Air Travel and Parking/Ground Transport
Self-parking at Central Parking, near the Omni Charlotte, is $12/day; valet parking is $20/day.

Housing
Room rates in the CHHSM room block are $129.00 per night for single or double occupany. Each additional person, of any age, is $20.00 per night. Hotel room rates are subject to applicable state and local taxes in effect at the time of check in. Check in time is 3:00 pm. Check out time is 12:00 pm.

Annual Meeting registrants must make their own hotel reservations here by calling 1-800-The Omni and use the code 16300104627 to receive the special room rate. The deadline for hotel reservations is February 13, 2008. After February 13, rooms may no longer be available at CHHSM's special room rate. The Omni Charlotte is located at 132 E.
