If your nomination is for one of the awards catagories, be sure to include a narrative (500 words or less, Word doc, or PDF file) outlining the nominee's accomplishments and contributions to your ministry and the impact of the nominee's efforts upon the ministry and/or persons served.

Recognition nominations (Retiring Chief Executive Officer, Significant Anniversary) do not require submission of a narrative.
